Investigation Summary
At 10:30 a.m. on August 5, 2011, an employee was installing new brake shoes on the rear drive axles of a 2006 Volvo model 780 tandem-axle semi tractor (fleet #414277). The rear axles of this semi tractor were in an elevated position supported by two American Forge & Foundry model 565E pneumatic/hydraulic jacks. In addition, all wheels and tires were removed from the rear axles. The employee was working beneath rear axles of the semi tractor striking part of the brake linkage with a hammer when the semi tractor fell off of the jacks. The employee's head was positioned beneath part of the air suspension system of the semi tractor, and when the truck fell the air suspension frame pinned the employee's head crushing it against the concrete floor. The truck was not supported by jack stands, and the truck was not otherwise restrained from movement because the parking brakes were disabled and the front tires were not chocked. The employee's skull was fractured by the falling truck, and the employee was killed.
