Investigation Summary
On July 20, 2011, an employee had been assigned to work a double shift at a foundry. During the employee's first shift, which would have been the third shift, he had been working to find a hydraulic leak in the hoses that ran under the furnace. The evening was hot and humid. He was working from a JLG for an hour, underneath the furnace trying to chip away metal that had hardened onto the hoses. During that time, the employee was wearing head protection, eye protection, hearing protection, foot protection, and foundry greens over his street clothes. After his work was completed, he decided to take a cold shower to help cool down. He began his second shift, but stayed in the air conditioned shower house for most of it. About four hours into his second shift, at approximately 9:50 a.m., he was found unresponsive on the floor of the shower house. He was taken to OSF hospital and pronounced dead due to a heart attack. The employee suffered from coronary atherosclerosis.
