Worker Is Fatally Crushed by Pipe Manifold Assembly

On March 31, 2014, Employee #1, employed by a trucking company, was working at a recycling yard, Nevada West Recycling. He was positioned near the passenger side and toward the front of a flatbed trailer truck. The flatbed trailer held a 45-foot long, 8,000-pound section of pipe manifold assembly that had been removed from a geothermal power plant. A Nevada West Recycling employee was operating a Gradall rough terrain forklift positioned on the driver side and toward the back of the flatbed trailer. The forklift made contact with the pipe manifold assembly, which caused it to roll off the passenger side of the flatbed trailer. Employee #1 was struck and fatally crushed by the falling pipe manifold assembly. This fatality was investigated by Nevada OSHA.
