Investigation Summary
At approximately 1:45 p.m. on February 24, 2014, Employee #1, a truck driver, was working from a tank trailer. As he climbed down from the trailer, he lost his balance and fell approximately 3 ft from the side of the tank trailer. Employee #1 sustained a fractured hip in the fall. He was transported to a medical center, where surgery was performed and he was then hospitalized.
