Investigation Summary
At approximately 4:00 p.m. on December 23, 2013, Employee #1, a cleaning worker, was cleaning a belt conveyor with a towel when the towel she was using to clean caught and pulled her hand and arm into the conveyor's point of operation. A coworker helped to free her from the conveyor belt and she was transported to an area hospital, where her left arm was amputated.
