Investigation Summary

At approximately 5:10 p.m. on February 21, 2013, Employee #1, a 58-year-old male cameraman (steadicam operator,) with NCIS Los Angeles/Paramount Pictures was standing near where a vehicle (minivan) struck a container. The vehicle collision caused the container to rapidly move with a clockwise rotation and come to rest few inches from where a second container and where Employee #1 was standing. The container caused a crushing injury to his torso, including ribs, liver, and lungs. Employee #1 was transported for treatment to Providence Holy Cross Hospital. The facility involved in the incident was a production company employing over 100 workers. Employee #1 had been working there for approximately four years, performing duties of a Cameraman/Steadicam Operator. At the time of the incident, Employee #1 was performing his regularly assigned task, using his steadicam to film a stunt. Employee #1 was filming the actual stunt at the time of the incident which was an intentional car crash on the set of NCIS-Los Angeles, where a van crashes into a cargo container and comes to a complete stop. The van was traveling approximately at 25-miles-per-hour and struck the first container that caused it to skid and pivot into a new trajectory toward the steadicam operator, who was standing adjacent to the second cargo container, and subsequently got pinned between two cargo containers.
