On June 8, 2012, an employee, of D & D Roofing and Sheet Metal, Inc., Sparks, NV, was working on a roof, removing roofing material. He stepped through an unstable and unsecured portion of the roof. He fell approximately 20 feet to the concrete floor below. He suffered unspecified injuries that required hospitalization.
