Investigation Summary
At approximately 11:00 a.m. on January 21, 2014, Employee #1 used an extension ladder to paint the kitchen walls of the dining hall. The ladder slipped over the plastic tarp that it was installed on and he fell approximately 20 feet to the ground, sustaining serious multiple injuries.
