Employee Is Injured When Fork Lift Tire Explodes

At approximately 9:00 a.m. on August 7, 2013, an employee, a mechanic, was inflating a tire on a forklift. The split rim tire was installed on a "Gerlinger" forklift, Model Number PH862, Serial Number 2432, with the capacity of 6,000 lbs. The fork lift was located in the yard. The left front tire was low with air. The employee was inflating the tire with a clip-on chuck with a less than 24-inch long hose to an in-line hand valve and gauge. Two lug nuts on the wheel rim were missing. As the employee was inflating the tire, it exploded suddenly knocking him down resulting in serious head injuries. A restraining device or safety cage was not used during inflation. The employee was hospitalized and treated for head injuries.
