Investigation Summary
At approximately 3:00 p.m. on June 30, 2011, Employee #1 was working near the loading dock. He was bent at his waste taking inventory when he fell off of the docks edge to the ground. He suffered a laceration to the head as a result of the fall but had a past medical history of heart problems. Employee #1 later died due to heart related complications.
