Investigation Summary
On August 25, 2011, Employee #1 was installing steel beams in an opening in a roof. The opening measured approximately 27 feet by 15 feet, and the distance from the opening to the floor below was approximately 15 feet 2 inches. Employee #1 was using a fall protection system, which was located approximately 4 feet from the opening in the roof. Employee #1 moved to the southernmost end of the opening with his double-legged lanyard and rope. Employee #1 fell through the opening in the roof. His lanyard, in combination with the rope grab system was too long, and Employee #1 hit the ground before the fall protection system was able to arrest his fall. Employee #1 landed on his right hip, and fractured his pelvis. Employee #1 was transported to a local hospital and was treated for his injuries. He was later transferred to a rehabilitation facility to continue his recovery.
