Employee Dies Falling Onto Concrete Floor

Employee #1 was using a pallet jack to carry one bundled pallet of envelopes, and was working from the floor level. Employee #1's order sheet showed he had to pick from a stack that was on the second tier. No one witnessed the accident, but there are foot prints on the pallet jack, boxes, and racks that would indicate Employee #1 climbed up on the rack to pick what he needed. At some point it would appear Employee #1 fell and injured his head on the concrete floor. When a coworker found him, Employee #1 had removed his shoes, some of his layered shirts, he had vomited, and was in the fetal position moaning. Employee #1 was taken to a local hospital, where he later died.
