Investigation Summary

Investigation Nr: 202634655
Event: 07/28/2011
Employee Is Burned in Gas Explosion

On July 28, 2011, Employee #1 was working in a kitchen area, converting a gas range from natural gas to propane. After completing the necessary steps for the conversion, Employee #1 lit the range to determine if it was functioning properly. An explosion and flash fire occurred, causing second-degree burns to Employee #1's face, arms, and chest. Emergency services were contacted, and Employee #1 was taken by helicopter to a nearby medical facility, where he was hospitalized and treated for his injuries. The accident investigation determined that the explosion and flash fire were as a resulted of a leaky gas pipe that was installed along the base of the wall. The propane vapor spread to the kitchen area and when the range oven was activated an ignition source was provided, causing the explosion and fire.
