Investigation Summary
At approximately 11:45 a.m. on December 28, 2012, Employee #1, a 42-year-old laborer employed by R & R Roofing Contractors Inc., was performing construction work at a single family residence. He was part of a crew replacing the roofing material. Employee #1 was on the roof and then descended. Coworker #1 was tending the hot tar bucket. The hot tar flashed, and as it started to burn Coworker #1 put the lid on the bucket to extinguish the fire. Employee #1 attempted to put out the fire by putting his foot on top of the bucket, but his foot instead went into the hot tar. Employee #1 was transported to the University of California San Diego Regional Burn Center, where he was admitted, treated for serious burns to his foot and right hand, and hospitalized for several months. The employer reported this event to Cal/OSHA at 11 a.m. on March 4, 2013.
