Employee Amputates Finger While Clearing Jammed Machinery

On May 8, 2012, Employee #1, a supervisor, was working on a Cumberland machine, which diced up foam into little pieces, and had become jammed. The machine was deenergized and locked out prior to entering it. Employee #1 was picking out scraps with his hands when he reached in between a stationary blade and the blade that rotates on the Cumberland machine. His body pressed against the rotary and inadvertently moved the rotary, which closed the gap between the two blades while his finger was in between them. Employee #1 sustained a partial left index finger amputation, and was hospitalized.
