Worker Dies after Falling from Ladder

On October 28, 2010, an employee was working as a laborer for a contractor primarily engaged in the installation of roofing, siding, and sheet metal work. He was working from a 32-foot extension ladder. He was standing on the ladder at an approximate height of 16 feet, near the eave of the roof. Coworkers of his were on the roof, removing the shingles and other roofing debris. As they removed the debris, they slid it down the roof to him. He would then grab the debris and throw it into the bed of their truck, which was parked in the driveway below. For some reason, the ladder on which the employee was working started sliding. He pushed off from the ladder and landed face-first on the sidewalk below. He sustained a fracture to his head, and he died shortly thereafter.
