Investigation Summary
At approximately 6:13 p.m., on December 21, 2011 Employee #1 of Silgan Plastics Corporation was walking through the yard, returning to the warehouse. Employee #2 was in process of unhooking a Kalmar model 4 by 2 yard tractor from a 52 foot enclosed trailer he had just parked at the loading dock. As Employee #1 crossed in front of the tractor, Employee #2 unhooked and began pulling forward and initiating a hard right-hand turn. Employee #1 was struck by the bumper and dragged underneath the truck, and eventually run over by the left rear dual wheel. Employee #1 received fatal injuries and died at the scene.
