Investigation Summary

At approximately 11:00 a.m. on June 8, 2011, Employee #1, a 62-year-old female with Regional Cardiology Associates Medical Group was the transcription supervisor, supervising three medical transcriptionists working in the same room. One of the coworkers was listening to a doctor's recorded notes through a computer headset and typing the notes on the computer. The coworker came to a word she could not understand in the recording and asked another corker, the transcriptionist seated at the computer to her right, to listen to the word for her. The worker unclipped her headset cord and passed the headset to the coworker so she could listen. However, they still could not understand the word, so they asked Employee #1 to come over and listen. Employee #1 did so, then put the headset down on the desk and turned to walk back to her desk on the other side of the room, but caught her foot on the headset cord and fell to the ground. The other employees came to her aid, at first, Employee #1 felt fine, but after a while, her left shoulder began to feel increasingly painful. So a coworker drove her to the medical clinic, then the emergency room where they learned that she had broken her shoulder. Employee #1 was not hospitalized for more than twenty four hours, but had surgery the following week.
