Employee Suffers from Heart Attack, Later Dies

At approximately 5:00 p.m. on July 5, 2011, Employee # 1 and coworkers were cleaning a bridge foundation pad of soil and rocks in preparation for the building of forms for a concrete pour operation. Employee #1 told his coworker he had difficulty breathing and need air. Employee #2 and Employee #3, assist Employee # 1 out of the pit to the tool truck. Employee #1 had heart attack and died at the construction site. A heart attack was suspected at the time and later verified with an autopsy.
