Investigation Summary

Investigation Nr: 200785095
Event: 07/17/2010
Employee Is Killed When Pulled Into Conveyor

On July 17, 2010, Employee #1 was operating a conveyor system at a railcar terminal. The equipment was a portable, self-powered load conveyor system used to off-load materials from railcars and upload them into dump trucks and storage silos. He was opening a railcar hopper bent-over next to a conveyor belt operating at 1,400 to 2,000 revolutions per minute. He tried pushing down on the hopper gate door latch and fell onto the belt. He was pulled under the two unguarded belt tensioners and up the inclined portion of the conveyor system. He was killed.
