Investigation Summary
At approximately 915 a.m. on December 23, 2009, Employee #1, a painting supervisor, was attempting to move an electrical whip or pigtail that was hanging from the ceiling so drywall finishing could take place. The whip was energized with 277 volts and was to be part of the buildings lighting circuit. He attempted to throw the approximate 8 ft whip back up and over some piping in the ceiling. The whip did not catch and fell back down, losing two of its wire nuts. He grabbed the whip again and was electrocuted when he contacted a ground source and the live conductor on the whip.
