Investigation Summary
On November 4, 2009, Employee #1 was painting a 400-ft-high cell communication tower, working from the top down. He was approximately 300 ft above the ground. When he repositioned his single fall protection lanyard to a lower location, his pelican hook positioning device disengaged from the structure. Employee #1 fell approximately 30 ft and struck a support bracket for guys. He grabbed a guy in the crook of his elbow and slid down the guy until he reached the ground. He was hospitalized, however no injured were indicated in the narrative.
