Investigation Summary
At approximately 1:35 p.m. on June 13, 2013, Employee #1, a machine operator, was cleaning out an extruder machine. The employee's right hand was caught in the machine's point of operation while it was spinning. Employee #1 was taken to St. Francis Hospital, where he was treated for a fractured right arm and later released.
