Investigation Summary
At approximately 3:05 p.m., on August 1, 2011, Employee #1 was walking from a customer counter to the back of the restaurant. Employee #2, who was standing, turned and stepped in his way. Employee #1 tried to avoid Employee #2 and made a quick turn towards the left when he lost his balance and fell to the floor on his right side. The accident caused multiple fractures on his right thigh which required a surgery and hospitalization for three days.
