Investigation Summary
On June 2, 2010 at approximately 5:00 a.m., Employee #1 sustained serious head injuries from an unknown source. Employee #1 stated that he was going to be moving hay with the tractor. Employee #1 stated that he usually stands on the second step of the ladder on the left side of the tractor, to start the tractor with the key. Employee #1 said that he did not remember how long it was before the emergency response team found him unconscience, by the tractor. Employee #1 was transported to a medical center.
