Investigation Summary
On November 1, 2010, Employee #1, a maintenance mechanic, was on an 8 ft., two-sided fiberglass stepladder adjacent to the exterior wall of a building replacing a light fixture. The ladder collapsed when the rivets that fastened the hinge to the ladder beams failed. Employee #1 fell to ground. He was hospitalized at St. Jude Medical Center for a fractured right wrist. There were no witnesses to the fall.
