Investigation Summary
On January 26, 2009, Employee #1, of Springdale Ready-Mix, was performing maintenance on a conveyor belt system. While performing maintenance, he leaned on an unopened, folding ladder to access the conveyor system. The ladder slipped on ice, and Employee #1 fell approximately 5 feet and struck his head on the concrete support pier. He was hospitalized and died from head wounds.
