Investigation Summary
At approximately 9:30 a.m. on October 14, 2012, Employee #1, a construction laborer, was working at a single-family residence undergoing exterior and interior remodeling. He was painting the exterior trim and helping a roofer. The roofer asked Employee #1 to move a 16 ft, single-section aluminum ladder from the east side of the property to the west side. He set up the ladder, which was missing both safety feet, onto the concrete driveway in front of the garage. As Employee #1 ascended the ladder, the bottom slid approximately 5 ft. He then fell to the driveway, striking his head. Employee #1 was hospitalized at Palomar Hospital for a head injury and an unspecified laceration.
