Investigation Summary
At approximately 11:35 a.m. on August 25, 2010, Employee #1 was installing a broken hydraulic fluid container on a Yale LP Forklift (Model NumberGLC050, Serial Number N568604). The supervisor had leaned two pallets together to support the forks up while the employee and a coworker removed the hydraulic fluid container. After the installation, the coworker left the employee to finish up alone. The employee went to start up the engine but found some fluid leaking. He stopped the engine and returned to the area beneath the forks and mast to investigate. While doing so, the forks and load guard came down and crushed the employee's forearms and wrist. He suffered a fracture to the left clavicle, fracture to both arms and wrist, and a deep laceration to left arm. Employee #1 was hospitalized for nine days.
