Employee Is Killed When Struck by Truck

On December 12, 2008, Employee #1 was preparing to unload waste water, in the ramp area, of a waste water treatment facility. A coworker of another truck completed unloading and began pulling away from the ramp. After pulling forward a little, he stopped and dismounted the truck. He walked to the rear of his truck and observed Employee #1 laying face up on the ground, bleeding from the eyes and nose. Employee #1 had been run over and died.
