Employee Is Asphyxiated by Carbon Monoxide

At approximately 3:00 p.m. on May 27, 2007, Employee #1 returned to a 15 feet by 15 feet 8 inches A-frame cabin that was his sleeping quarters at the company. The cabin was provided by the employer because the site was in a remote location. The overnight low was approximately 34 degrees Fahrenheit. The next morning, May 28, he did not show up for his scheduled work time of 9:00 a.m. At 9:15 a.m., he was found dead, lying in bed in the cabin with a Williams Model Number 2001611 liquid propane (LP) heater running. The medical examiner ruled the death to be due to carbon monoxide poisoning. The heater, installed in the cabin three years prior to the accident, was found to have the following deficiencies: the vent safety shutoff device was not connected, the ventilation and ductwork was not installed properly, and the heat exchanger was corroded and cracked.
