Investigation Summary

Investigation Nr: 201491834
Event: 09/03/2008
Employee's Foot Is Runover by Forklift

At approximately 10:30 on September 3, 2008, Employee #1 was working as a data processor for a citrus processing and packing company. She had placed a written tag onto a, approximately 6-ft high, stack of fruit bins. The fruit bins were to be moved within the packing house later by a forklift. As she came out from behind the bins with the stack to her left, she first looked to her right as she stepped out and into the pathway of a traveling forklift. The forklift was coming from her left and across her front. The forklift rolled over her right foot causing serious crushing injuries. She was transported to Selma District Hospital for treatment but later transferred to Fresno Regional Medical Center to undergo surgery. Employee #1 was hospitalized.
