Investigation Summary

Investigation Nr: 202359055
Event: 12/05/2006
Employee Fractures Wrist When Caught in Vise

Employee #1 operates a Make-On machine, which permanently installs fittings on 1-in.; 1.25-in.; 1.5-in.; and 2-in. diameter sprinkler system pipes, for sale as prefitted pipe. At the time of the accident, Employee #1 was changing the master box. The master box is where the end of the pipe with the fitting gets inserted during the fitting installation process. Employee #1 was changing the master box and his left hand was in the open jaws of the pneumatic vise. The vise holds the opposite end of pipe when a fitting is installed on a length of pipe. Employee #1's upper left arm inadvertently hit the lever that opens and closes the vise, causing his left hand and wrist to be caught in the jaws of the vise. Employee #1 sustained a fractured wrist and was transported to the hospital for evaluation and treatment. Employee #1 was released later that night.
