Event: 12/11/2007
Employee killed when struck by falling refractory material

On December 11, 2007, Employee #1 was spraying a new, castable-refractory lining on the North Hot Metal Mixer at #2 Q BOP US Steel. He was taking a break and was sitting on the bottom of the hot metal mixer, which was tilted at approximately 20 degrees from vertical, when a 4 ft by 4 ft by 14 in. thick, 2,000 lb piece of refractory fell from approximately 6 ft above, striking him on the back of the head. An ambulance crew from US Steel rescued Employee #1 from the mixer and transported him to Broadway Methodist Hospital--Northlake Campus, in Gary, IN, where he was pronounced dead on arrival.
