Employee's Finger Is Caught Between Racks and Amputated

On August 12, 2010, Employee #1, a store clerk, was working at a retail food store. A customer asked him to get a 5 gallon empty plastic bottle located on the top shelf, about 7 ft high. The employee climbed the rack to retrieve the bottle. As he was climbing down, his little finger got caught in the space between racks. Employee #1's little finger on his right hand was partially amputated, and he was hospitalized.
