Investigation Summary

Investigation Nr: 201353273
Event: 01/08/2007
Employee Is Injured in Fall from Ladder, Later Dies

On January 2, 2007, an employee and a coworker were removing Christmas decorations from a patio area. To remove garlands from overhead wooden beams, the employee used a six feet tall aluminum step ladder, Davidson Model Number W-2212-066 that was manufactured in October 2004. The ladder was in good repair with no defects and was properly placed. After removing some garlands, the employee stepped from the ladder onto the ground. He apparently lost his balance and fell backwards. He may have missed the bottom rung of the step ladder as he was descending. He struck his shoulders and head on the concrete floor of the patio. The employee received a cut on the back of his head, but did not lose consciousness, and was described as being alert and oriented. He was taken to a local hospital, where he received a cat scan. He was diagnosed with a subdural hematoma. Over time, his condition worsened, and on January 8, he died after undergoing surgery to correct the subdural hematoma.
