Investigation Summary
On March 10, 2007, Employee #1 was delivering roofing materials to a roof using a ladder. The general contractor placed a piece of wood, 4 ft wide and 2 ft deep, over the gutter to protect it from disfigurement by the ladder. The wood was held stationary by placing 8 to 10 roof tiles on the interior side of the wood piece. A Louisville ladder, Model Number L-2323-16, 16 ft, Type III, 200 pounds, was placed against the piece of wood that overhung the edge of the gutter by 7 to 8 inches and was not secured. Employee #1 grabbed the two remaining pieces of roof tile and placed them on his shoulder as he started his decent. While holding on to the ladder with one hand, the ladder shifted to the left, causing Employee #1 to fall approximately 10 ft. He fell backwards toward the dirt ground and suffered a broken neck vertebra. He was hospitalized and diagnosed as a quadriplegic.
