Investigation Summary
At approximately 5:00 p.m. on May 24, 2005, an employee was siding a second story addition while a ladder used by a coworker was moving. While standing on the ladder, the coworker fell into the employee's ladder. The employee was killed as a result of a fall of 15 feet from an extension ladder.
