Employee Is Killed in Fall

On March 10, 2005, Employee #1 was erecting a system scaffold approximately 70-ft high. He was wearing a safety harness with two lanyards, both of which had shock absorbers. The employee had attached the southern end of the horizontal runner/bearer and was walking on a lower horizontal runner/bearer to attach the northern end. Employee #1 slipped and fell, grabbing the partially attached runner/bearer. The runner/bearer then swung back towards the south, causing Employee #1 to slip off the end and fall approximately 50 ft to the concrete floor. He sustained head injuries, and was killed. Employee #1 was not tied off at the time of the fall.
