Investigation Summary
On September 29, 2004, Employee #1, an apprentice, was working in a vocational-technical program. He had erected a ladder at the rear of a dwelling under construction in order to install a bath fan. The top of the ladder was approximately 18 ft above ground level, while the base of the ladder had been placed approximately 10 ft back from the dwelling, at an extreme angle. Ground conditions were muddy, and the ladder was not secured. The ladder slipped, but did not collapse, causing the employee to fall to the soft muddy ground. Employee #1 sustained bruised ribs and was disabled for approximately 6 days.
