Employee Dies after Being Crushed by Vehicle

At approximately 8:30 p.m. on January 16, 2009, Employee #1 was changing a flat tire on the front passenger side of a 2008 Dodge Grand Caravan. He had jacked up the right side of the vehicle and removed the flat tire. He was lying underneath the front center area of the vehicle, while trying to remove the spare tire. The vehicle fell off the jack and crushed him. He was hospitalized and died four days later.
