Investigation Summary
At approximately 9:30 a.m. on June 9, 2004, Employee #1 was felling trees. He cut down a Red Oak tree, which became lodged between two Poplar trees. To remedy the situation, he felled one of the Poplar trees and then began to make a notch into the other. As he notched the third tree, the Oak tree began to fall. Employee #1 attempted to move out of the way, by fleeing up a 20- to 30-degree slope away from the falling tree, but tripped and fell due to undercut on the ground. The Oak tree struck Employee #1 in his head and back, pinning him underneath the tree. A coworker that was working more than 200 feet from Employee #1 noticed that he had not seen Employee #1 come out of the woods for a long time and went to check on him. Employee #1 was found pinned under the tree with his chain saw still running. Emergency services were called. Employee #1 was pronounced dead due to his injuries.
