Investigation Summary
At approximately 3:00 p.m. on January 5, 2005, Employee #1 was working at a recreational vehicle and truck repair shop located in Colton, California. He was performing a post-maintenance clean up operation on a coach recreational vehicle, using a rag to wipe excess fuel from around the vehicle's (energized) generator unit with a CD 7000 blade fan type. The compartment containing the generator unit was located on the side of the recreational vehicle and generator was approximately 3 ft long and 2 ft high. The employee's right hand was pulled into the fan blade, and his right ring finger was amputated when it came in contact with the blade. Employee #1 was transported to U.S. Healthworks then subsequently transferred to Redlands Community Hospital, where surgery was performed and he was hospitalized for four days. The employee stated he was not aware that the fan was a suction type fan, and that he might not have turned off the generator had he known the amount of suction would have pulled his right hand into fan blade. He also said that he felt he had adequate training to do his job. He had twenty two years experience as a mechanic.
