Employee's Eye Is Struck by Flying Grinding Disk Piece

At approximately 2:30 p.m. on June 26, 2007, Employee #1, a welder, was grinding a 2 by 2 piece of metal tubing using his own grinder with a cut-off disk. He was wearing a plastic face shield, Huntsman Face Saver, Model Number 8154. He was not wearing any eye protection under the face shield. His equipment was not appropriate for this task and should have been using a cutting torch. Employee #1 did receive safety training regarding safety eye protection, welding, grinding, forklift and other topics per employer's written records. The cutting disk broke and a piece broke through his face shield and cut his left eye. Employee #1 was hospitalized.
