Investigation Summary

Investigation Nr: 307183657
Event: 07/24/2006
Employee's Fingers Are Crushed by Tortilla Machine

At approximately 10:30 a.m. on July 24, 2006, Employee #1, a sanitation employee, was cleaning the tortilla-making machine, Model number MMFTHP3232, in the production area. He was trained to clean the machines with the conveyor belt of the press operating. The conveyor belt was located under the press, which moves the balls of dough, and then the press moves up and down to flatten the dough into a tortilla shape. Normally, the machine operators turn off the press to lower it for cleaning. Employee #1 attempted to clean machine number 14 and saw that the press was approximately 10 inches above the conveyor belt. He turned on the conveyor belt, but found that it was jammed, so he placed his hand on it to assess the problem. The press inadvertently came down on his fingers and crushed them. He was hospitalized for his injuries.
