Employee injures leg when struck by forklift

At approximately 10:38 a.m. on March 26, 2004, Employee #1 was stacking rolls onto pallets with his coworker. The coworker, a forklift operator, was backing up a Model 30-Hyster clamp lift forklift from a container. As the operator backed up the forklift onto the loading dock area, the forklift struck Employee #1. The forklift ran over his leg, fracturing bones within the leg. Employee #1 was transported to a medical center for treatment and hospitalized for postoperative care until approximately 15 days later, when he was discharged from the hospital. Causal factors in the accident were that the forklift operator did not ensure employees were clear from the movement of forklift operations and look in the direction of travel prior to the commencement of the work.
