Employee Is Killed in Fall from Scaffold

On November 22, 2004, Employee #1 was working off a two section high rolling scaffold, with a 15 ft extension ladder placed on top of it and leaned against a wall to paint the 30 ft high wall. The foreman on the ground was supposed to lock the wheels on this rolling scaffold prior to Employee #1, accessing the scaffold and ladder. This was not done and as Employee #1 was accessing the ladder on top of scaffold, the scaffold wheels moved. This resulted in Employee #1 losing his balance and falling approximately 30 ft to the floor below the scaffold. Employee #1 fractured his left ankle and left thumb and was hospitalized.
