Employee crushed between fork lift and truck

On January 5, 2004, Employee #1 of National Wood Products Company (NWPC) was discovered dead by his coworkers. He was pinned between the company's delivery truck and a propane powered Mitsubishi fork truck. Employee #1 was not authorized to work on site until January 5, 2004 due to the holiday weekend. The last day that the employees were to be on site was December 31, 2003.
