Investigation Summary
On January 28, 2004, an employee was riding on the side of the last of three loaded rail cars. The cars were being pushed down the rail that had an accumulation of snow and ice. The rear car derailed and struck the nearby brick building. The employee was pinned between the side of the car and the building. He was killed.
