Employee falls through unprotected floor opening and dies

On March 16, 2004, Employee #1 and three coworkers, framing carpenters, were all involved with construction of a single family dwelling. They were working in the bonus room over the garage area. Two coworkers were working at the left side wall of the garage stapling plywood to the wall. The owner and Employee #1 were working on the ceiling area. The other coworker was in the driveway area on the ground cutting materials as needed. Employee #1 was walking to the rear of the bonus area over the garage to nail down a rafter plate with an air nailer. He stepped or fell into an unprotected floor opening and fell approximately 21.58 ft to the concrete floor below and died.
